Back-to-School Forms

Welcome to a new year of learning and growth! You can now complete your back-to-school paperwork online—no need to print, fill out, and bring it in. Each form is sent straight to the school office, and you’ll get an email confirmation. Prefer paper? You can still pick up copies in the school office.

A few things to know

The permission slip is completed on paper — pick one up in the school office. A permission slip and a record of your child’s last check-up must be updated every year.
Immunization records must be received before a child starts attending, and updated whenever your child gets shots. New students should also have a copy of their birth certificate on file.
The Health Assessment has a section only your child’s physician can complete. Have them email it to connect@tlgreeley.com or send it in.
Tuition can be paid through the office or online. Already enrolled? Make a tuition payment.

Getting the year started. Classes begin August 12. There is a parent meeting on Monday, August 10 from 6:00–6:30 p.m., with open-house time to visit classrooms and meet the staff from 5:30–6:00 p.m. and 6:30–7:00 p.m. You can bring paperwork that evening, but payments won’t be taken then.
