Two parts to this form. You complete the parent section below. The Health Care Provider section must be completed by your child’s physician after a check-up and emailed to connect@tlgreeley.com or brought to the office. A record of the child’s last check-up must be updated annually.
Signing today records your authorization with today’s date. This authorization expires 365 days after the signature date.
